New Client Information
At Concierge Animal Hospital we welcome new clients and look forward to establishing a long and rewarding relationship with both you and your pet.
We serve veterinary clients throughout the coastal cities of Los Angeles (please see our service area map for more details), and accept all clients by appointment, Monday through Friday. Contact us online via our appointment scheduler or by phone at (310) 818-3294 to schedule your pet’s appointment. Please fill out our new client form (included above) prior to your visit.
If possible, we also ask that you have your pet’s previous medical records or a number for the previous provider available so that we may call to request a copy on your behalf.
